Adoption Application

If you are interested in adopting a cat or dog, you will first need to fill out an adoption application. Applications are available at the adoption clinics, or you can download it below. Please print and fill out the appropriate application. You can then submit it one of three ways:

  • scan and email it to:
  • send it in the mail to:

  • drop it off at one of our adoption clinics
